How to Claim Your AirTranquil Purchase Through Insurance

If you've recently invested in your sleep health with AirTranquil, you may be eligible for reimbursement from your insurance provider. While we do not bill insurance companies directly, we want to make the "out-of-network" claim process as painless as possible.

Follow this step-by-step guide to submit your paperwork and get your money back.

Step 1: Secure Your Out-of-Network Claim Form

Most major insurers require a specific form for out-of-network medical equipment claims. You can usually find these on your provider's member portal.

Quick Links & Addresses for Major Providers:

  • Anthem: (Regional addresses vary; check your member portal or the back of your ID card).
  •
  • United Healthcare: PO Box 5230, Kingston, NY 12402-5230.
  •
  • Cigna: Attn: Claims Service Dept, PO Box 46270, Eden Prairie, MN 55344.
  •
  • Aetna & Humana: Typically require mailing to the specific address listed on the back of your insurance ID card.
  •

Step 2: Fill Out the Details

Accuracy is the key to avoiding delays. You will need to provide your personal information alongside our provider details:

Field Required Information

Tip: When listing your items, break them down individually. If you bought a "Mask Bundle," list the mask and the headgear as separate line items to match the HCPCS codes below.

Step 3: Use the Correct HCPCS Codes

Insurance companies use specific alphanumeric codes to identify medical equipment. Match the items on your AirTranquil invoice to these codes:

Machines & Humidifiers

  • E0601: CPAP or Auto-CPAP (APAP) Machine
  •
  • E0470: Bi-Level/Bi-PAP (Without backup rate)
  •
  • E0471: Respiratory assist device, Bi-Level pressure (Bi-PAP) capability, (With backup rate)
  •
  • E0562: Heated Humidifier
  •

  • A7030: Full Face Mask
  •
  • A7034: Nasal Mask or Nasal Pillows
  •
  • A4604: Heated Tubing (Integrated element)
  •

  •
  • A7046: Water chamber for humidifier

Step 4: Final Review and Submission

Before you seal the envelope, double-check your work. Missing signatures or a missing invoice are the most common reasons for claim rejection.

  1. Attach Your Invoice: You must include the original AirTranquil receipt showing you paid in full.
  2.
  3. Sign and Date: Ensure all member signatures are complete.
  4.
  5. Keep a Copy: Always snap a photo or make a photocopy of your completed claim form and invoice for your personal records.
  6.
  7. Most Forms will have the address on the form, or you can usually find it on the back of your membership card.
  8.
